The aim of this study was to compare the differences in the quality of analgesia by patient-controlled analgesia (PCA) and nurse-controlled analgesia (NCA) for post-caesarean section analgesia.350 women who undertake elective cesarean section were assigned to the three groups. Group I (n=200), IV-PCA morphine; group II (n=100), IV-PCA methadone; group III (n=50) NCA morphine. Data collected during the 24 h observation period included visual analog scale (VAS) pain and patient satisfaction scores, the incidence of nausea and vomiting, severe sedation and pruritis.VAS pain scores for each time at which it was evaluated were higher for NCA group than other groups. Also patient satisfaction was significantly increased in the IV-PCA Group as compared with group III. The prevalence of pruritis was higher for NCA group than other groups.In post caesarean section, PCA morphine or methadone improves 24-h VAS compared with NCA.

Stereotypes About People Living with HIV: Implications for Perceptions of HIV Risk and Testing Frequency Among At-Risk Populations.

Although research continues to demonstrate that HIV stigma is associated with decreased HIV testing, the psychological processes implicated in this association remain unclear. The authors address this gap by differentiating between the HIV stigma mechanisms of stereotypes, prejudice, and discrimination. They hypothesize that HIV stereotypes specifically, more so than prejudice or discrimination, are associated with HIV testing among at-risk populations. Ninety-three HIV-negative people receiving methadone maintenance therapy at a clinic in the northeastern United States participated by completing a survey. Results demonstrated that HIV stereotypes are associated with HIV testing via the mediator of perceived HIV risk. As hypothesized, prejudice, discrimination, and objective HIV risk were not associated with perceived HIV risk. Differentiating between HIV stigma mechanisms in future work can provide critical insight into how to intervene in HIV stigma to increase HIV testing and improve HIV prevention among at-risk populations.

[Acute cerebellitis in children: regarding different etiologies].

Acute cerebellitis is usually infectious, post-infectious or post-vaccination. Rarer causes include toxic ones. The clinical presentation isusually benign, however, severe cases with brainstem compression may present as coma. Regarding three pediatric clinical cases, theauthors make a brief review on this entity, with particular emphasis on its causes and imaging findings.Case Reports: Three cases of pediatric acute cerebellitis are presented- post-viral, after acute intoxication with carbon monoxide andafter accidental ingestion of methadone.Discussion: The etiopathogenic mechanisms underlying the imaging changes found in each of the three cases presented are discussed.Conclusion: An imaging examination confirms the diagnosis of cerebellitis and is important to monitor severe cases. MR is more sensitivethan CT in the evaluation of the cerebellar parenchyma.
