It means that the facility is not requiring that the power of attorney and designation of health care surrogate be agreed to is not being required for treatment or admission to the facility.

    The person signing the contract is affirming that the the REASON they are signing is not because they are trying to get treatment that has previously been refused to them.

    As far as I can tell from the context, it is illegal for a health care facility to make signing this contract a pre-requisite to getting treated.

    This is just a check that you aren’t being forced to sign it, in order to obtain treatment.
