Open Question: BAD MIX-UP!!! Switched from 20mg Methadone TID to 15mg Morphine TID…….?

I was stable on 20mg Methadone TID (60mg total per day) for over a year. My Dr. quit her practice and I was forced to find a new Dr. armed WITH only an HMO (trouble right there). When I got to my appointment I was out of meds a getting opiate sickness (it was getting worse by the minute). When I arrived the front desk told me that my PCP office failed to fax over all of my records by the day before my appointment and therefore I could NOT be seen by the Dr. that day. I told them my situation pleading with them…no avail. I asked if they could call me an ambulance. She gave me a dirty look and said “No”. I called the ambulance myself and they gave me an IV of dilaudid and a percocet Rx until I could find a Dr. So, after about a week, I go to a pain clinic and explain what happenned. The Dr. gave me 15mg morphine TID and I got VERY sick again. It was late Friday by the time I realized I was in trouble and had to leave the Dr. a message to say how sick I was etc… As a result, I took more of the morphine than Rx’d. I went to see him ASAP the next week on Tuesday. He was upset with me….blah blah so I ended up back on methadone after suffering cuz he only gave me a weeks’ worth of Morphine at the low dose. So, after suffering and missing work being on morphine, He gave me 7 tablets of 10mg methadone for the next week, I was sooooo sick I STILL couldnt work, eat etc….In fact, I lost 20 pounds in about 10-14 days b/c of it. Does this sound like a reasonable dosing change for someone in my situation? Or does it sound like my new Dr. was under-prescribing and being overly conservative and just letting me suffer? I finally went to a different PCP Dr. and he put me back on the 75 mcg fentanyl patch. 12 hours later I was feeling fine again. Jim
